My name is Kari Pruim, RN, BSN and I am the full-time Registered Nurse at Salk Elementary School.  My main job as a School Nurse is to keep Salk students safe and healthy and ready to learn.  Here are some things parents can do to assist me:

 

*Please help me keep your child’s school medical record up to date.  Share any and all medical conditions, diagnosis and medications with me and notify me any time things change.

*Please be sure your student’s immunizations are current. The Lake County Health Department offers free immunizations for children. 

*Take a couple of moments to discuss frequent and thorough hand washing with your student, and encourage them to cough and sneeze into their elbow rather than hands.  These simple acts can help prevent the spread of infection!

*If your child has any medical considerations, please make sure to fill out a medical update form every school year.  Having this form filled out by you and your doctor can be very helpful in the event that your child becomes ill during school hours. 

*If your child requires medicine to be kept at school, you and your doctor must fill out the medication administration form.  This form expires at the end of each year and must be resubmitted each year.  All medication must be brought to school in its original, pharmacy-labeled container by an adult.
